Job summary

You Matter is seeking a compassionate Senior Housing Support Practitioner to join our step-down services in the community, focused on helping residents achieve greater independence and move on toward independent living. This role involves managing a large caseload and working in a solo capacity with a strong emphasis on safety and safeguarding.

You will work with residents on resettlement, community engagement, wellbeing, and tenancy sustainment, ensuring a safe, welcoming environment.

Qualifications

• Experience delivering person-centred support in housing or community settings.
• Ability to manage health and safety and safeguarding in a building.
• Capable of working independently with minimal supervision.

Responsibilities

• Manage a large caseload and provide person-centred support focusing on resettlement, wellbeing, and tenancy sustainment.
• Support residents to develop skills and independence for move-on from the service.
• Oversee day-to-day building management, health and safety, safeguarding, and a welcoming environment.
• Be comfortable with lone working and make decisions as situations arise; have a visible presence at Nightingale House.
• Attend team meetings and may facilitate supervision sessions for peers.
