You will provide practical relocation support, develop individual plans, and connect residents with health, housing and social services. Lived experience with relocation is valued, and you will contribute to program improvement and collaborative care.
We are seeking a well-rounded Support Worker who is passionate about helping vulnerable older people navigate relocation, settle into their new homes and stay connected to their community.
As a not-for-profit organisation, Better Health Network supports people and families at different stages of life and works with communities to strengthen health and wellbeing. We believe that better health and wellbeing is built around community.
Joining our Better Homes Better Health program, you’ll support public housing residents transitioning to new social housing. Through regular outreach, practical assistance and service connections, you’ll help residents access the support they need throughout their move.
Your responsibilities will include:
You’ll bring strong engagement skills, a person-centred approach and a genuine commitment to supporting vulnerable older people during change. You’ll be empathetic, resilient and adaptable, with the ability to build relationships and work collaboratively to support positive client outcomes.
To be successful, you’ll bring:
Lived experience of relocation is highly regarded, but is not essential. We also welcome applicants who bring relevant professional experience, transferable skills and a genuine commitment to this work.
Knowledge of local community services and proficiency in a language other than English would also be valued.
At BHN, you’ll join a values-driven organisation that genuinely cares about its people and the communities it serves. We offer:
This role is based at 240 Malvern Road, Prahran, with a strong community-based outreach focus and work at additional sites as required.
We value Accountability, Collaboration, Respect and Courage, and we strive to create an inclusive environment that welcomes people from all backgrounds, including those from Aboriginal and Torres Strait Islander, culturally and linguistically diverse, and LGBTQIA+ communities.
Pre-employment requirements include reference checks, a National Police Check, Working with Children Check, Aged Care Worker Banning Order Check and evidence of the right to work in Australia. An international police check is required where applicable
