The Tulip Care Network is seeking a Support Worker to provide person-centred care to older people and those with disabilities in the community. You will help clients maintain dignity, safety and independence while living at home and staying connected with their community.
Responsibilities include personal care, mobility support, meal prep, domestic tasks, companionship, and documenting progress. Relevant qualifications and a valid licence are required.
The Support Worker provides person-centred care and support to older people and or people with disability living in the community. The role focuses on assisting clients to maintain their dignity, wellbeing, safety, and independence while remaining in their own homes and engaged in their community.
Assist clients with personal care tasks, including showering, dressing, grooming, toileting, and continence support
Support mobility, transfers, and safe movement within the home
Encourage clients to maintain independence and choice in daily activities
Provide assistance with light household tasks, including cleaning, laundry and bed making
Support meal preparation in line with dietary requirements and preferences
Assist with grocery shopping and essential errands as outlined in the care plan
Provide companionship and emotional support
Encourage and support participation in social, recreational, and community activities
Observe, monitor, and report changes in a client's physical, emotional, or cognitive condition
Accurately document care provided, observations, and client progress
Certificate III in Individual Support (Ageing, Home & Community Care) or equivalent
Current First Aid and CPR (or willingness to obtain)
Current Police Check and relevant aged care/ NDIS screening checks
Valid driver's licence and access to a reliable vehicle (if required)
Strong interpersonal and communication skills
Empathy, patience, and respect for older people
Ability to deliver person-centred, strengths-based care
Ability to work independently and as part of a team
Experience supporting older people in a community or care setting (preferred)
Experience working in home-based or non-residential aged care environments (desirable)
