Job summary

Accord Care Qld Pty is seeking an experienced Disability Support Worker to assist a participant living in Cheltenham, Victoria, at an SDA property. The role involves complex support for a wheelchair and bed-bound individual with MS, requiring catheter care and two-person transfers, while maintaining dignity and safety.

You will follow participant-specific plans, document accurately, and work with a multidisciplinary team to ensure continuity of care and adherence to NDIS standards.

Qualifications

• Certificate III or IV in Individual Support or equivalent.
• Current NDIS Worker Screening Check.
• Current Working with Children Check, where required.
• Current First Aid and CPR certificates.
• Current medication-assistance training and manual-handling/hoist-transfer training or demonstrated experience.

Responsibilities

• Provide respectful personal care, including toileting, continence, dressing.
• Administer and assist with medications per plan.
• Support two-person transfers and manual handling.
• Safely use hoists, slings and transfer equipment.
• Support positioning, pressure-care and daily living activities.
• Observe changes and escalate concerns per procedures.
• Complete clear progress notes and handover documentation.
• Collaborate with participant, family, and clinical staff.
