Job summary
ACCORD CARE seeks an experienced Disability Support Worker to assist a participant in Cheltenham, Victoria, who is MS and wheelchair/bed-bound, requiring catheter and two-person transfers. You will follow tailored support plans, manage personal care, continence, mobility, and daily living activities with a safety-first approach.

Qualifications
- Certificate or equivalent experience in individual support.
- NDIS Worker Screening Check and WWC where required.
- Current First Aid and CPR certificates.
- Current driver license and work rights in Australia.
- Experience with two-person transfers and manual handling.
Responsibilities
- Provide personal care, continence support and hygiene.
- Perform two-person transfers using hoists per plan.
- Assist with catheter, sleep positioning and pressure care.
- Administer medication as per plan and ACCORD CARE procedures.
- Document progress notes and handover information clearly.
