Job summary

ACCORD CARE is seeking an experienced Disability Support Worker to assist a participant in Cheltenham, Victoria. The role involves complex support needs, including MS, catheter care, and two-person transfers, with strict adherence to care plans and safety protocols.

You will work with a collaborative team to provide personal care, mobility support and daily living activities, ensuring dignity and independence for the participant. Commitment to documentation and escalation protocols is essential.

Qualifications

• Certificate III/IV in Individual Support, Disability or equivalent or relevant experience.
• Current NDIS Worker Screening Check.
• Current Working with Children Check if required.
• Current First Aid and CPR certificates.
• Current medication-assistance training.
• Current manual-handling/hoist-transfer training or relevant experience.
• Valid Australian driver licence.
• Evidence of work rights in Australia.

Responsibilities

• Provide personal care including toileting, continence support and hygiene.
• Perform catheter care per plan and procedures.
• Support a wheelchair- and bed-bound participant with daily living activities.
• Carry out two-person transfers and manual handling safely.
• Use hoists, slings and transfer equipment as required.
• Assist with positioning and pressure-care needs.
• Ensure participant comfort, dignity and safety in care.
• Provide medication assistance per plan and ACCORD CARE procedures.
• Support mealtime management per guidelines.
• Observe health changes and escalate concerns per procedures.
• Follow participant plans and clinical guidance consistently.
• Complete progress notes, incident reports and handovers clearly.
• Maintain a safe and organized care environment.
• Collaborate with family, team and clinicians.
• Follow NDIS Practice Standards and ACCORD CARE policies.
