Job summary

ACCORD CARE is seeking a Disability Support Worker to assist a participant in Braybrook, Victoria. The role involves supporting a person with cerebral palsy, severe intellectual disability and dysphagia, with personal care, mobility, mealtime safety and daily living tasks.

You will follow participant support plans, clinical guidance, swallowing guidelines and emergency procedures, maintaining structured routines, accurate documentation and continuity of care within a supportive team.

Qualifications

• Certificate III/IV in Individual Support or Disability or equivalent experience.
• Current NDIS Worker Screening Check and Working with Children Check where required.
• Current First Aid and CPR certificates; Medication-assisted training; Manual-handling training or demonstrated relevant experience.

Responsibilities

• Provide respectful personal care including toileting, continence support, dressing and hygiene.
• Provide safe mealtime assistance following dysphagia and aspiration-risk procedures.
• Recognise choking signs and respond per emergency procedures.
• Provide medication assistance per care plan and ACCORD CARE procedures.
• Perform ceiling-hoist transfers safely per manual-handling plan.
• Support safe use of powered wheelchair and shower commode.
• Assist with positioning, mobility and pressure-care.
• Use non-verbal communication to understand participant preferences and needs.
• Follow Positive Behaviour Support strategies and early-intervention approaches.
• Complete progress notes, incident reports and handover documentation.
• Follow evacuation procedures for non-ambulant participants.
