Job summary

ACCORD CARE is seeking a Disability Support Worker to assist a participant living in Cheltenham, Victoria at an SDA property. The role involves complex physical needs and requires two staff for transfers, catheter care, and adherence to support plans to ensure safety and dignity.

The successful candidate will have a Certificate III or IV in Individual Support or equivalent, current checks, a valid driver licence, and a commitment to documentation and escalation protocols.

Qualifications

• Minimum 2 years in disability and/or aged care (preferred).
• Experience with MS and complex physical support needs is highly regarded.
• Catheter care experience valued.

Responsibilities

• Provide respectful personal care incl. toileting, continence, dressing and hygiene.
• Provide catheter care per plan and procedures.
• Assist wheelchair and bed-bound participant with daily living activities.
• Perform two-person manual handling and transfers safely.
• Safely use hoists, slings and transfer equipment.
• Support positioning and pressure-care needs.
• Mediate medication assistance per plan.
• Document progress notes and handover clearly.
• Escalate concerns per procedures.
• Follow participant support plans and clinical guidance.
