Zusammenfassung

Accord Care in Brisbane, QLD is seeking experienced Disability Support Workers for a role supporting an individual with complex mental health needs, including schizophrenia. The position involves assisting with daily living, mobility and community access, with shifts from morning through to an attached afternoon/sleepover roster.

You’ll work closely with the participant and the broader team, following PBSP guidelines and ensuring safety and respectful, person-centred care at all times.

Qualifikationen

• Certificate III in Individual Support, Disability Support or related field.
• Experience supporting individuals with schizophrenia, mental health conditions or psychosocial disabilities.
• Experience with behaviours of concern, including aggression.
• Current open Australian driver’s licence.
• Reliable personal vehicle with insurance for community access.
• NDIS Worker Screening clearance and Blue Card, as applicable.
• Clear English communication and ability to follow support plans.
• Calm, patient and respectful approach to challenging situations.
• Ability to work collaboratively while maintaining professional boundaries.
• Availability to work morning shifts and/or attached afternoon/sleepover shifts.

Aufgaben

• Provide person-centred support that promotes independence, dignity and choice.
• Support an individual with complex mental health needs, including schizophrenia and psychosocial disability.
• Assist with daily living activities and personal care as required.
• Support safe mobility and transfers in accordance with relevant support plans.
• Assist with community access and appointments, including transporting the participant in your own vehicle.
• Implement behaviour support strategies in line with the participant's Interim Positive Behaviour Support Plan (PBSP).
• Respond to behaviours of concern, including verbal and physical aggression and absconding risks.
• Follow health, safety, incident reporting and documentation procedures.
• Maintain professional boundaries and communicate effectively with the participant and the wider support team.
