Job summary

ACCORD CARE serves participants with SIL, in-home care and disability supports across NDIS. We seek a reliable Disability Support Worker for a Redbank Plains-based participant with diabetes, mental health needs and behaviours of concern. You will monitor BGL, administer insulin, and follow care plans with a compassionate, person-centred approach.

The role requires sleepovers and a mix of morning/afternoon shifts; nursing experience is valued and training will be provided where needed.

Qualifications

• Experience supporting participants with complex diabetes management needs.
• Competency in BGL monitoring and recording.
• Experience administering insulin via pen or syringe, safely and appropriately.
• Experience testing and recording ketone levels.
• Understanding diabetes care plans and signs of emergencies.
• Experience supporting people with mental health needs and behaviours of concern.
• Strong communication, observation, documentation and problem-solving skills.
• Ability to stay calm and person-centred in challenging situations.
• Willingness to work a mix of morning, afternoon and sleepover shifts.

Responsibilities

• Provide person-centred support with personal care and daily living activities.
• Monitor and record BGL and ketone levels per diabetes management plan.
• Administer insulin safely where trained and approved.
• Recognise signs of hypo/hyperglycemia and escalate as required.
• Support mental health needs and respond to aggression with de-escalation.
• Follow diabetes care plans, medication procedures and PSBP.
• Maintain progress notes, clinical records and incident reports.
• Promote dignity, independence and safety while collaborating with the team.
• Comply with ACCORD CARE policies and NDIS Practice Standards.
