Job summary

Ambition Health Group is seeking an experienced Disability Support Worker to assist an adult SIL participant with progressive neurological disability, complex care needs, and mobility and communication challenges. You will provide hands-on support, ensure dignity and safety, and work as part of a multidisciplinary team.

The role emphasizes calm, patient, reliable care, adherence to plans and policies, and engagement with community participation and daily activities.

Qualifications

• NDIS Worker Screening Check is required.
• Current First Aid and CPR certifications.
• Disability, Individual Support or Community Services qualification, or equivalent experience.

Responsibilities

• Provide personal care including showering, dressing and continence care.
• Assist transfers, positioning and safe manual handling.
• Support wheelchair use and community participation.
• Monitor skin integrity and pressure care.
• Support mealtime, dysphagia-safe feeding and nutrition.
• Coordinate with allied-health professionals and other staff.
• Maintain dignity, choice and independence of the participant.
