Job summary

ACCORD CARE in Mango Hill recruits Disability Support Workers to provide 24/7 support in home and community settings within our supported independent living arrangements.

The role includes personal care, positive behaviour support, documentation, and collaboration with team leaders to implement daily service plans. We value reliability, clear communication, and a client-centered approach in line with NDIS guidelines.

Qualifications

• Experience supporting people with mental health needs and complex behavior.
• Ability to communicate clearly and build relationships with clients and team.
• Proven problem-solving skills in home and community care settings.

Responsibilities

• Supporting people with personal care and day-to-day activities in the home and out in the community.
• Providing positive behaviour support to clients.
• Completing documentation through shift notes, incident reporting and data collection.
• Contributing to team goals to achieve the best client outcomes.
• Working with team leader to implement and review daily service plans.
• Maintaining professional relationships and adhering to NDIS and company code of conduct.
• Being responsive, reliable and accountable to clients and the team.

ACCORD CARE is trusted in the provision of person-centered disability support services to NDIS participants. We provide Supported Independent Living, In-home care, Community Participation, Short-Term Accommodation, Community Nursing, and Allied Health Services.

About the role

We are looking for Disability Support Workers in our supported independent living arrangements in Mango Hill to provide 24/7 support in the home and out in the community.

The support worker’s role and responsibilities will include:

• Supporting people with personal care and day-to-day activities in the home and out in the community
• Providing positive behaviour support to our clients
• Completing appropriate documentation through shift notes, incident reporting and data collection
• Contribute to the team’s overall objective of providing the best outcomes for our clients
• Work with your team leader to implement and review plans around daily service provision
• Maintain a professional relationship with clients and adhere to the NDIS and company code of conduct
• Above all, be responsive, reliable and accountable to our clients and your team
