Job summary

Magistrates' Court of Victoria in Ballarat Central VIC is seeking an experienced Case Manager to deliver high-quality, person-centred support to participants in the Court Integrated Services Program (CISP). You will provide holistic case management to individuals with complex needs, including mental illness, homelessness, and substance use, to support recovery and stability.

Qualifications

• Diploma level qualification in social work, psychology, welfare, AOD, or related field (or equivalent).
• Experience in case management of clients with complex needs is required.
• Knowledge of mental health, disability, AOD, or homelessness is highly desirable.

Responsibilities

• Deliver case management using a strengths-based, evidence-informed approach.
• Build strong relationships with clients and stakeholders.
• Conduct assessments and develop person-centred support plans.
• Identify and manage risk to clients and others.
