Court Services Victoria is seeking an experienced Case Manager to deliver high quality, person-centred support to participants in the Court Integrated Services Program (CISP). You will provide holistic case management to individuals with complex needs, collaborating with clients and stakeholders to create tailored support plans that promote recovery and stability.

Qualifications

  • Diploma level minimum in case management with forensic clients or equivalent experience.
  • Degree in social work, psychology, welfare, AOD or related field is highly desirable.
  • Strong knowledge of mental health, disability, AOD or homelessness as applicable to complex needs.

Court Services Victoria provides corporate services and facilities to support the performance of the judicial, quasi-judicial and administrative functions of Victoria's courts and tribunal, Judicial College and Judicial Commission.

About the Role

We are seeking an experienced Case Manager to deliver high quality, person-centred support to participants in the Court Integrated Services Program (CISP).

You will provides case management support to individuals participating in the CISP program who present with a range of complex needs, including mental illness and/or cognitive impairment, alcohol and other substance use issues, homelessness, family violence, and/or limited social supports, to support recovery and stabilisation.

You will provide holistic case management, working collaboratively with clients and stakeholders to develop and implement tailored support plans that promote recovery and reduce risk.

Key responsibilities:

  • Deliver case management using a strengths-based, evidence-informed approach.
  • Build strong, trusting relationships with clients and stakeholders.
  • Conduct assessments and develop person-centred support and treatment plans.
  • Identify and manage risk to clients and others.
  • Contribute to service quality, continuous improvement, and positive client outcomes.

About You

You are a skilled and empathetic professional with experience supporting clients with complex needs. You will bring:

  • Tertiary qualification (minimum Diploma level; Degree highly desirable) relevant to case management with forensic clients, such as social work, psychology, welfare, alcohol and other drug (AOD), or a related field, and/or equivalent experience.
  • Demonstrated experience or capability in case management of clients with complex needs, with strong knowledge and/or experience in at least one of the following areas: mental health, acquired brain injury, disability, alcohol and substance use, family violence, or homelessness (highly desirable).
  • Detailed knowledge of the mental health and/or disability service system, with a demonstrated ability to build and maintain effective stakeholder relationships.
  • Sound understanding of evidence informed practice models and frameworks for working with clients experiencing complex needs, including family violence, disability, mental health, and/or AOD.
  • Excellent case formulation and written communication skills.
