Job summary

Court Services Victoria is seeking an experienced Case Manager to deliver high quality, person-centred support to participants in the Court Integrated Services Program (CISP). You will provide holistic case management to individuals with complex needs, including mental illness, homelessness, or substance use, to promote recovery and reduce risk.

Qualifications

Tertiary qualification (minimum Diploma; Degree desirable) in case management with forensic clients.
Experience in case management of clients with complex needs; knowledge in mental health, brain injury, disability, AOD, or homelessness.
Knowledge of mental health/disability services; ability to build stakeholder relationships.
Understanding of evidence-informed practice for complex needs, including family violence, disability, mental health, and AOD.
Excellent case formulation and written communication skills.

Responsibilities

Deliver case management using a strengths-based, evidence-informed approach.
Build strong, trusting relationships with clients and stakeholders.
Conduct assessments and develop person-centred support and treatment plans.
Identify and manage risk to clients and others.
Maintain accurate, compliant documentation.
