Job summary

Court Services Victoria is seeking an experienced Case Manager to deliver high quality, person-centred support to participants in the Court Integrated Services Program (CISP). You will provide holistic case management to individuals with complex needs, collaborating with clients and stakeholders to create tailored support plans that promote recovery and stability.

The role involves assessments, risk management and strong stakeholder engagement to drive positive client outcomes within the justice

Qualifications

• Diploma level minimum in case management with forensic clients or equivalent experience.
• Degree in social work, psychology, welfare, AOD or related field is highly desirable.
• Strong knowledge of mental health, disability, AOD or homelessness as applicable to complex needs.

Responsibilities

• Deliver case management using a strengths-based, evidence-informed approach.
• Build strong, trusting relationships with clients and stakeholders.
• Conduct assessments and develop person-centred support and treatment plans.
• Identify and manage risk to clients and others.
• Contribute to service quality and continuous improvement.
