Job summary

City of Geelong is seeking a dynamic Coordinator Community Care Programs to lead the delivery and continuous improvement of community care services including Food Services, Home Maintenance, Intake and Service Establishment. You will work with a new leadership team to shape customer‑centred, financially sustainable service models that respond to community needs in Geelong.

Qualifications

• Tertiary qualification in Community Services, Health, Human Services, Aged Care or related discipline, or extensive equivalent experience.
• Experience leading complex operational programs within aged care or related regulated services.
• Experience leading service improvement, program development and workforce planning.

Responsibilities

• Lead the design, implementation and review of program models while ensuring compliance, quality and financial sustainability.
• Ensure services are delivered in line with the Aged Care Act, Standards, funding agreements and procedures.
• Analyse incidents, feedback and trends to identify issues and drive continuous improvement.
• Lead, coach and develop direct reports to deliver safe, inclusive and accountable Community Care programs.
• Provide leadership through reform and operational change, ensuring priorities and performance expectations are understood.

About the role

The City is entering an exciting period of transformation as we prepare for the future of aged care and community services. We are seeking an energetic, values driven leader to join our newly established Community Care leadership team and help shape the next chapter of service delivery for our community.

As Coordinator Community Care Programs, you will lead the delivery, innovation and continuous improvement of key community care services including Food Services, Home Maintenance, Intake and Service Establishment, and to be developed and shaped Community Programs. Working as part of a newly established leadership team, you will help shape contemporary service models that are customer-centred, financially sustainable, compliant and responsive to the evolving needs of our community.

You will work closely with the Care Services Lead and Business Development and Compliance Coordinator to translate community insights, emerging opportunities, performance data and aged care reform into practical improvements that enhance service quality and outcomes.

This is a hands-on leadership role where no two days are the same. You'll lead operational teams, oversee workforce planning, manage budgets and contractor partnerships, champion innovation and drive a culture of accountability, learning and continuous improvement. Most importantly, you'll help ensure our services continue to make a meaningful difference in the lives of older people and carers across our community.

Key responsibilities include:

• Lead the design, implementation and review of program models that respond to community need, including emerging or unmet need, while maintaining compliance, quality and financial sustainability.
• Ensure services are delivered in accordance with the Aged Care Act, Aged Care Quality Standards, funding agreements, Council policies, reporting requirements and relevant operational procedures.
• Analyse incidents, complaints, feedback, missed services, performance data and service trends to identify systemic issues and drive continuous improvement.
• Lead, coach and develop direct reports to deliver safe, inclusive, customer-focused and accountable Community Care programs.
• Provide leadership through reform, service redesign and operational change, ensuring staff understand service priorities, performance expectations and their role in delivering sustainable outcomes.
• Develop, implement and review operational plans that support service sustainability, budget accountability, funding requirements, workforce capacity and community need.

• Relevant tertiary qualification in Community Services, Health, Human Services, Aged Care or a related discipline, or extensive equivalent experience.
• Demonstrated experience leading complex operational programs within aged care, health, community services, local government or a comparable regulated service environment.
• Demonstrated experience leading service improvement, program development, workforce planning and operational change.
• Experience managing service delivery, contractor relationships, compliance requirements, budgets and workforce matters in a complex service environment.
• Demonstrated people leadership skills, including the ability to lead, coach and develop teams through change, reform and service improvement.
• Proven ability to use service, workforce, financial and performance data to identify risks, service gaps, improvement opportunities and sustainable program responses.
• Strong analytical, problem-solving and decision-making capabilities, with the ability to translate issues into practical actions and sustainable service improvements.

Preferred candidates will require a Working with Children's Check and be willing to undergo a National Police Check as part of the recruitment process (funded by the City).
