Job summary

Social Futures Ltd in Tweed Heads is seeking a Program Worker, Connecting Home, for a part-time fixed-term position four days per week. You will work with clients to provide intake, assessment and brief intervention supports as part of a multidisciplinary team.

You will develop skills in assessments, maintain professional boundaries, and contribute to wrap-around housing and social support services while building inclusive communities.

Qualifications

• Tertiary qualifications or equivalent experience in health, psychology, social work, social science or social welfare, health promotion or similar
• Experience providing high quality customer-centred individual case management with people.
• Excellent written and oral communication, interpersonal and negotiation skills, with the ability to communicate sensitively and effectively with all people to ensure effective relationships
• Demonstrated well-developed organisational, time management and administrative skills with the ability to plan, prioritise and meet deadlines
• Demonstrated experience in maintaining professional boundaries while engaging in person-centred work

Responsibilities

• Work collaboratively with people to resolve their housing issues.
• Provide intake, assessment and brief intervention supports to people as they access the Connecting Home service.
• Be part of a multidisciplinary team providing holistic support to people.
• Develop your skills and knowledge in undertaking assessments and implementing brief evidence-based interventions
