Job summary

Social Futures in Tweed Heads is seeking a Part-Time Program Worker, Connecting Home, to support wrap-around housing services and strengthen family, social and cultural connections. You will work with a multidisciplinary team to provide intake, assessment and brief intervention supports, and contribute to holistic person-centred care while pursuing ongoing professional development and wellbeing benefits.

Qualifications

• Qualified in health, psychology, social work or related field.
• Experience providing customer-centred case management.
• Excellent written and oral communication and interpersonal skills.
• Strong organisational and time management abilities.
• Ability to maintain professional boundaries in person-centred work.

Responsibilities

• Collaborate with individuals to resolve housing issues.
• Provide intake, assessment and brief intervention supports.
• Be part of a multidisciplinary team delivering holistic support.
• Develop skills in assessments and brief evidence-based interventions.
