Job summary

Social Futures in Lismore is seeking a Program Worker for the Family Connect and Support team on a part-time fixed-term basis. The role focuses on working with families, children and young people to promote wellbeing through person-centred, evidence-based approaches and goal planning, including outreach and home visits.

Qualifications

• Strong communication and ability to work collaboratively with service providers and stakeholders.
• High-level understanding of risk and the complexities involved in safeguarding and family supports.
• Ability to work with families to build confidence and empower positive changes.
• Attention to detail to collect and ensure data integrity for reporting requirements.
• Willingness to participate in supervision, case reviews and audits.
• Ability to meet mandatory reporting requirements in line with policy.

Responsibilities

• Case coordination and supporting families through strength-based capacity building.
• Conduct strength-based assessments considering the whole family system and cultural context.
• Develop clear, achievable plans to connect families with the right supports.
• Provide flexible service delivery in outreach capacity across the Northern Rivers, including home visits.
• Provide warm handovers to support meaningful service engagement.
• Recognise and respond to early indicators of harm to prevent statutory intervention.
• Maintain timely and accurate case notes and client records per requirements.
• Support culturally responsive practice for Aboriginal families and diverse communities.
• Develop understanding of CAFS specifications, risk frameworks and local supports.
• Engage families with warmth, empathy and professional boundaries.
• Represent Family Connect and Support in universal settings to engage isolated families.
