Social Futures Ltd in Tweed Heads is seeking a Program Worker, Connecting Home, for a part-time fixed-term position four days per week. You will work with clients to provide intake, assessment and brief intervention supports as part of a multidisciplinary team.

You will develop skills in assessments, maintain professional boundaries, and contribute to wrap-around housing and social support services while building inclusive communities.

Qualificações

• Tertiary qualifications or equivalent experience in health, psychology, social work, social science or social welfare, health promotion or similar
• Experience providing high quality customer-centred individual case management with people.
• Excellent written and oral communication, interpersonal and negotiation skills, with the ability to communicate sensitively and effectively with all people to ensure effective relationships
• Demonstrated well-developed organisational, time management and administrative skills with the ability to plan, prioritise and meet deadlines
• Demonstrated experience in maintaining professional boundaries while engaging in person-centred work

We are seeking a Program Worker, Connecting Home to join our team on a Part-Time (4 days a week) Fixed Term basis in Tweed Heads to help make a positive social change, together.

The Connecting Home team works closely with other stakeholders to ensure wrap-around support and understands the importance of strengthened family, social and cultural connection.
