Job summary

Qualifications

• Experience in direct care within disability, aged care or community services.
• Strong interpersonal and communication abilities to build trust and respond to diverse needs.
• Understanding of safeguarding, duty of care and relevant policies.

Responsibilities

• Provide person-centred assistance in clients' homes and communities.
• Support daily living activities: personal care, meal prep, medication prompts (within scope) and domestic tasks.
• Facilitate social and community participation and accompany to appointments and activities.
• Maintain accurate records, follow care plans and safeguarding procedures.
• Communicate regularly with families, coordinators and health professionals to ensure consistent support.
