Job summary

Home Caring in Sydney, NSW is seeking a Community Worker to support people living with a disability and frail aged individuals. The role focuses on maximizing living, social and recreational skills, promoting inclusion, independence, decision making and personal choice through tailored services.

You will understand client needs, provide personal care and household assistance, assist with appointments, and build links with community resources while maintaining documentation in client management

Qualifications

  • Portuguese or Spanish speaker is highly desirable
  • Living in Northern beaches is highly desirable

Responsibilities

  • Understanding the client's needs and adapting services individually, making sure they have appropriate assistance to live independently as much as possible, within principles of Duty of Care, Respect and Dignity
  • Supporting clients according to their needs, making sure they have assistance for personal care, household tasks, shopping, and mealtime support, helping them to progress towards their goals and develop their general living skills
  • Providing emotional support to clients and their families
  • Supporting and monitoring the health care needs of clients, engaging them with allied health providers, make relevant referrals and deal with suppliers as per care plan
  • Assisting clients to organize their appointments and/or social activities making sure they can access the community and recreation programs
  • Facilitate care assessment, care planning and review with clients and family, according to client deadlines and develop and monitor individual plans and budgets with regular reviews
  • Development and maintenance of all documentation in client management systems updated within legislated NDIS and CDC requirements
  • Develop links in the community to source new referrals and allied health partners
  • Working pro-active to aim the retention of existing consumers and reporting feedback, to be addressed properly, in different levels within the organization
  • Developing and maintaining effective relationships with customers and team members through clear communication

Job description

The Community Worker is responsible for support services aimed at maximizing the living, social and recreational skills of people living with a disability and those who are frail aged. Community inclusion, independence, decision making and personal choice should be promoted through these support, with a focus on individual needs and goals.

Key responsibilities

About you
