Qualifications

• Qualification in Community Services, Social Work or equivalent.
• Experience in a community services or support role.

Responsibilities

• Assess client needs and connect them with health, housing, employment, education, and social services.
• Interview clients to identify challenges and develop support plans.
• Plan and deliver education, training, and support programs to promote wellbeing.
• Collaborate with families, health professionals, and government agencies.
• Refer clients to service providers and follow up for continuity of care.
• Monitor progress and evaluate programs, recommending improvements.
• Maintain confidential client records and reports.

The role involves providing structured support services to individuals and families, assisting them to access appropriate community resources and services in line with assessed needs.

Key Duties and Responsibilities

• Assessclient needs and provide tailored support to access health care, housing, employment, education, and social services.
• Interview clients to identify social, emotional, financial, and personal challenges and develop appropriate support plans.
• Plan, develop, and deliver community education, training, and support programs to promote wellbeing and independence.
• Work collaboratively with families, communityorganizations, health professionals, and government agencies to provide integrated, client-centeredservices.
• Refer clients to relevant service providers and ensure follow-up to support continuity of care.
• Monitor client progress and evaluate the effectiveness of programs and interventions, recommending improvements where required.
• Maintain accurate and confidential client records, case notes, and reports in line withorganizationaland regulatory requirements.
• Promote social inclusion, equity, and respect for cultural and individual diversity.
• Advocate for clients’ rights and support their participation within community and service systems.
• Support clients in developing life skills, building independence, and engaging with the community.

Selection Criteria

• A relevant qualification in Community Services, Social Work, or a related discipline, orequivalent relevant experience
• Demonstrated experience in a community services or support role
• Sound communication, interpersonal, andorganizationalskills
• Ability to work both independently and as part of a team
