Job summary

Business Nest is seeking a compassionate NDIS support worker to assist participants across our registered service areas with life-stage support, daily tasks and community participation. You will help develop life skills, support independence and ensure participant choice and dignity are upheld, in line with NDIS practices.

The role requires strong communication, reliability and the ability to work sensitively with participants and their support networks.

Qualifications

  • AQF Certificate IV or higher qualification in Community Services, Disability, Social Services, Youth Work or a closely related discipline.
  • At least 1 year of relevant experience in community services, disability services or a comparable client-support role.
  • Demonstrated experience assessing or responding to client support needs, implementing individual support plans, facilitating community participation and promoting independence.
  • Experience liaising with families, support coordinators, allied health professionals and other community/service providers.
  • Experience monitoring participant progress and maintaining accurate case notes, progress records and incident documentation.
  • Sound understanding of person-centred practice, NDIS requirements, participant choice and control, safeguarding, privacy and dignity of risk.
  • Current First Aid/CPR, NDIS Worker Screening Check and Australian driver’s licence, or ability to obtain the required clearances/certifications before commencement.

Responsibilities

  • Assess participants’ individual support needs, goals and circumstances and assist in implementing person-centred support strategies and programs.
  • Support participants to develop independence, life skills, confidence and social inclusion in accordance with their individual support plans and NDIS goals.
  • Facilitate access to community, recreational, educational, social and other services and activities relevant to participants’ needs and goals.
  • Identify appropriate community resources and services and assist participants with referrals and connections to additional support where required.
  • Monitor participants’ progress, identify changes in needs or circumstances, and report outcomes and concerns to relevant stakeholders.
  • Liaise with participants, families, support coordinators, allied health professionals, community organisations and other service providers to coordinate appropriate supports.
  • Provide practical assistance with daily living and community access where required as part of implementing individual support plans.
  • Maintain accurate participant records, progress notes, incident reports and other documentation in accordance with NDIS and organisational requirements.
  • Promote participant choice, control, dignity, independence and safeguarding while complying with NDIS Practice Standards, WHS, privacy and organisational policies.

Job description

This is a client-facing position within a registered NDIS provider. The role requires a strong commitment to person-centred support, participant choice and control, privacy, dignity and safeguarding. The successful applicant will support participants across the organisation’s registered service areas, including life-stage and transition support, personal activities, travel/transport, daily tasks/shared living, development of life skills, household tasks, community participation and group/centre activities. The position requires reliability, integrity, sound communication skills and the ability to work sensitively with people with disability and their families/support networks.
