Job summary

Pearl Home Care - Sydney Inner West is seeking a Community Worker to provide person-centred support to older Australians living at home. You will assess client needs, coordinate services, implement support plans and advocate for clients to promote independence and safety.

You will liaise with healthcare professionals, maintain case notes, prepare reports, and participate in care planning to ensure services meet quality standards.

Responsibilities

• Conduct comprehensive client needs assessments and identify physical, emotional, social and environmental support requirements.
• Develop, implement and regularly review individual care and support plans in consultation with clients, families and healthcare professionals.
• Coordinate delivery of home care, community care and allied health services to meet individual client goals.
• Liaise with general practitioners, hospitals, allied health professionals, community organisations and government agencies regarding client care.
• Refer clients to appropriate health, welfare, disability, housing and community services.
• Advocate for clients to ensure access to suitable supports and community resources.
• Monitor client wellbeing and reassess changing care requirements.
• Identify risks affecting client safety and elevate concerns in accordance with organisational policies.
• Maintain accurate case notes, assessments, incident reports and client records.
• Assist clients to increase independence, community participation and social engagement.
• Provide education to clients and families regarding available services, funding and community resources.
• Participate in multidisciplinary case conferences and contribute to care planning.
• Ensure services comply with the Aged Care Quality Standards, organisational policies and relevant legislation.
• Prepare reports and documentation required by management and government funding programs.
