Qualifications

• Diploma or higher qualification in Community Services, Community Development or related discipline.
• At least one year of post-qualification experience in community services or support.
• Knowledge of community support systems and referral pathways.
• Experience with case management is preferred.
• Strong organisational and time-management skills.
• Excellent verbal and written communication skills.
• Ability to build positive relationships with clients, families and advocates.
• Commitment to person-centred support, dignity, inclusion and wellbeing.

Responsibilities

• Assess clients’ individual needs, develop personalised support plans and provide guidance on welfare, housing, health and community services.
• Interview clients to identify social, emotional, financial and personal challenges.
• Support clients to access relevant government agencies and welfare services.
• Monitor client progress, maintain case notes and review support strategies.
• Assist clients with daily living activities and community participation.
• Coordinate with families, carers and external service providers as needed.
