Job summary

Vis-Ability Support Services in Balmain NSW is seeking a Community Worker to join our team supporting people with disability and mental-health-related needs. We provide person-centred support, occupational therapy, support coordination and recovery coaching services across Sydney.

You will assess clients' needs, develop appropriate support plans and promote independence and community participation, while liaising with families, carers and health professionals to access health and community

Qualifications

• Relevant qualification in Community Services, Disability, Mental Health, Welfare or a related field.
• Experience supporting people with disability and/or mental-health-related needs.
• Strong communication, organisational and problem-solving skills.
• Ability to work independently and collaboratively within a professional team.
• Current NDIS Worker Screening Check and Working with Children Check (where applicable).

Responsibilities

• Assess clients' needs and help develop appropriate support plans.
• Provide person-centred assistance to promote independence and community participation.
• Support clients with daily living activities, social participation and access to community services.
• Liaise with families, carers, healthcare professionals and other service providers.
• Assist clients to access health, welfare, housing, employment and community resources.
• Monitor client progress and maintain accurate case notes and service records.
• Follow NDIS requirements, workplace policies and the NDIS Code of Conduct.
• Promote clients' rights, dignity, choice and self-determination.
