Sovereign Lives Victoria is seeking passionate Community Social Workers to provide holistic, culturally safe support to individuals and families in Melbourne. You will help clients access essential services, promote independence, and achieve goals for those living with disabilities, mental health conditions, and complex needs.

Responsibilities include in-home support, advocacy, and education through workshops, while collaborating with community groups and government bodies to ensure cohesive

Competenze

• Qualified in Social Work, Community Development, Human Services, or related field.
• Demonstrated experience with Aboriginal and Torres Strait Islander communities.
• WWCC and NDIS Worker Check current.
• Current First Aid and CPR certification.
• Valid driver's licence and access to insured vehicle.

Mansioni

• Conduct assessments to identify needs, goals, and strengths to develop personalised plans.
• Provide practical in-home support, emotional well-being assistance, and promote independence.
• Facilitate access to essential services, healthcare, education, and social programs.
• Plan and deliver educational workshops, community engagement, and group sessions.
• Monitor progress, review support plans, and report changes and outcomes.
• Refer clients to external providers when additional support is required.
• Advocate for clients' rights, dignity, and access to services.
• Conduct research on community needs and best practices to enhance delivery.
• Liaise with internal teams and external partners to ensure cohesive support.
• Maintain accurate client records and ensure confidentiality.
