Support Plus 4 You Pty Ltd is seeking a proactive Community Worker to join our team in Heatherton, VIC. You will meet clients, assess needs, and coordinate practical support to enhance wellbeing and independence.

You will develop education and training programs, refer clients to housing, health, employment and welfare services, and work with local groups to address community issues. A minimum of two years' experience is required.

Qualifikationen

• Minimum 2 years of experience in community services or related field.
• Ability to understand clients' needs and provide appropriate guidance.
• Good communication and interpersonal skills with clients, families and service providers.
• Good organisational and problem-solving skills, including record keeping.

Aufgaben

• Meet clients to understand their needs, concerns, and personal circumstances.
• Identify suitable community services and support options for clients and their families.
• Develop and coordinate support, education, and training programs to help clients improve wellbeing and independence.
• Help clients access housing, health, employment, education, and welfare services.
• Refer clients to government departments, welfare organizations, and other service providers when further assistance is required.
• Work with community groups, government agencies, and other organizations to address local community issues.
• Support young people, families and people with disabilities by providing guidance and helping them access appropriate services.
• Encourage clients to develop daily living skills, participate in community activities, and become more independent.
• Identify the needs of the local community and help organize suitable programs and services.
• Monitor clients' progress and keep accurate records of their support and development.
• Promote awareness of available community services and encourage people to use appropriate support resources.
• Prepare reports, funding applications, and other documents to support community programs and services.
