Job summary

Recruit Shop Pty Ltd in Brisbane, Australia is seeking a CHSP Intake & Service Coordination Officer to manage initial intake, onboarding and ongoing CHSP services. You will coordinate with rostering and care workers to tailor services for clients, ensuring smooth onboarding and ongoing support.

The successful candidate will bring aged care or community services experience, strong communication and documentation skills, and proficiency with client management systems and Microsoft Office.

Qualifications

• The candidate has aged care or community services experience including CHSP/Home Care Packages/Support at Home.
• Understanding of service coordination, onboarding and basic scheduling.
• Strong organisational and communication skills with attention to documentation accuracy.
• Confidence explaining aged care programs to clients and families.
• Proficiency with client management systems and Microsoft Office.
• Valid driver’s licence, Australian work rights and a National Police Check.

Responsibilities

• Coordinating CHSP services in line with program guidelines and policies.
• Managing client intake and onboarding, including sign-up documentation.
• Clearly explaining CHSP and Support at Home services to clients and families.
• Coordinating new services and changes with the rostering team.
• Communicating service requirements to care workers.
• Monitoring changing client needs and supporting service reviews and referrals.
• Maintaining accurate client records and compliance documentation.
• Supporting reporting, quality activities and general administration.
