Lifesquad is seeking a dedicated Disability Support Worker to help participants maximize living, employment, and social skills through person-centered support and community inclusion.

The role requires flexible availability across Monday to Sunday on a casual basis, with training during shifts. You will work with clients to promote independence, safe personal care, and meaningful participation in everyday activities while adhering to policies and documenting progress.

Formación

• Experience in disability support and personal care roles.
• Proven ability to communicate clearly with clients and families.
• Knowledge of safety, manual handling and client privacy.

Responsabilidades

• Support clients to participate in age-appropriate activities and tasks including employment, in home support and community access.
• Support clients to access activities and outings in the community.
• Provide direct care assistance to each client in accordance with their service plan and as directed.
• Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality.
• Monitor the safety and well-being of the clients and report any concerns to the Manager.
• Provide 1:1 or small group training and support in response to individual client needs.
• Ensure completion of appropriate documentation and record relevant information on support provided in client notes and communication book as required.
• Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
• Work with clients, family/carers, relevant case managers and other professionals to support the client's individual goals.
• Assume the role of key worker for designated clients, including the preparation of detailed daily shift notes placed into the Connecteam system on the clients' progress.

The appointed Disability Support Worker is responsible for implementing support services aimed at maximizing the living, employment, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.

About you

• Available to work Monday to Sunday on a casual basis.
• Be available during the week to have training whilst on shifts (buddy shift system) for the purposes of delivering effective and sufficient assistance to meet the client's needs.
• Proven and effective communication skills and understanding of the job role.
• Proven experience in manual handling and personal cares.
• The ability to engage and build excellent rapport with clients.
• Experience in working with clients with challenging behaviours.
• Proven excellent time management and good punctuality.
• Qualifications and certificates in disability/individual supports, mental health as well as years of experience are essential to this job role.
• Current Working with Children's Check, Full Australia Drivers Licence, Current First Aid and CPR, Third party car insurance and registration, NDIS Orientation Module completion, and NDISW work screening completion required.
