Job summary

Lifestyle Solutions is seeking a compassionate Disability Support Worker in Albion Park Rail, NSW. You will assist customers to become more independent at home and in the community, supporting personal care and daily living tasks.

Responsibilities include mentoring, coaching independent living skills, administering medication per plans, documenting handovers and incident reports, and helping customers access community programs. A current driver’s license and NDIS clearance are required.

Qualifications

• Be willing to work a variety of shifts including mornings, afternoons, evenings, awake shifts, sleepovers and weekends.
• Current driver’s license is a mandatory requirement.
• Current NDIS Worker Screening Clearance or willingness to obtain.
• Current First Aid Certificate (HLTAID003).
• Formal qualifications in Disability or Community Services would be advantageous.

Responsibilities

• Mentoring and skill development to help customers explore interests.
• Coaching independent living skills (cooking, domestic, social).
• Administering medication and following individual support plans.
• Completing clear handover notes and incident reports.
• Providing personal care when required (toileting, showering, feeding, dressing).
• Helping customers engage with community programs and activities.
• Community access responsibilities as required.
• Maintain basic computer/data entry and record keeping.

Lifestyle Solutions provide innovative and flexible person-centered support for people living with a disability. Support Worker to assist a customer to become more independent, in their own home and in the community.

As a Support Worker your responsibilities will include:

• Mentoring and skill development – guiding customers to explore and develop their personal interests/hobbies
• Coaching independent living skills such as cooking, domestic and social skills
• Administering medication and adherence to individual support plans
• Completing clear, concise and factual handover notes and incident reports
• Providing personal care such as toileting, showering, feeding and dressing when required
• Helping customers engage with their local communities and culture through access to Community Participation Programs and recreational activities
• Personal care is a must, and must be comfortable and knowledgeable in doing so.
• Epilepsy management knowledge is essential.
• Community Access will also be a requirement at certain times.
• Strong understanding of mental health conditions (depression, anxiety, schizophrenia, mood disorders).
• Understanding of mental health crisis management.
• Basic computer skills (data entry, record keeping, scanning, printing).
