Job summary

Lifestyle Solutions in NSW (Bella Vista) is seeking a dedicated Case Worker to support up to six children and young people in Out-of-Home Care. You’ll lead or implement case plans, coordinate services with care teams, and monitor progress toward long‑term outcomes, all while embedding therapeutic strategies.

This role requires a Diploma in Community Services or related field, experience with children and young people, a trauma‑informed, strengths‑based approach, and a current driver's licence.

Qualifications

• Hold a minimum Diploma in Community Services or a related field.
• Experience in case work with children and young people.
• Trauma-informed, strengths-based approach to your work.
• Ability to build strong relationships with diverse stakeholders.
• Current unrestricted driver's licence and ability to meet probity requirements.

Responsibilities

• Lead or implement case and cultural plans based on your state.
• Work with care teams to embed therapeutic strategies.
• Coordinate services, meetings and key stakeholders.
• Monitor progress and help keep case goals on track.
• Support permanency, transitions, life story work and family connections.
