Job summary

Lifestyle Solutions is seeking a Case Worker to support children and young people in Out-of-Home Care, delivering trauma-informed, therapeutic care and collaborative planning to achieve positive outcomes.

You will lead case plans, coach frontline staff, and work with families and external agencies to ensure aligned services and robust documentation in a culturally respectful setting.

Qualifications

• Diploma in Community Services or related field (or studying toward).
• Proven case management and stakeholder engagement experience.
• Knowledge of trauma-informed and therapeutic care practices.
• Strong documentation, organisation and compliance skills.
• Current driver’s license and probity clearances.

Responsibilities

• Lead and review case plans to achieve therapeutic, cultural, and individual goals.
• Coach and support frontline staff to embed therapeutic strategies in everyday care.
• Engage with families, carers, and external agencies to ensure aligned and positive outcomes.
• Maintain accurate documentation and ensure compliance with relevant standards and legislation.
• Promote safe, nurturing, and culturally respectful environments for children and young people.
• Support the delivery of trauma-informed, evidence-based care practices across services.
