Job summary

Lifestyle Solutions is seeking a Case Worker to support children and young people in Out-of-Home Care. You will lead therapeutic case management, coach frontline staff, and collaborate with families to create meaningful outcomes that keep young people safe and thriving.

You'll work in a trauma-informed environment, focusing on documentation, compliance, and culturally respectful care. A Diploma in Community Services, Youth Work, or Social Work is preferred, with ongoing professional development

• Knowledge of trauma-informed and therapeutic care practices.
• Strong documentation and compliance capabilities.

Responsibilities

• Lead and review case plans to achieve therapeutic, cultural, and individual goals.
• Coach and support frontline staff to embed therapeutic strategies in everyday care.
• Engage with families, carers, and external agencies to ensure aligned and positive outcomes.
• Maintain accurate documentation and ensure compliance with relevant standards and legislation.
• Promote safe, nurturing, and culturally respectful environments for children and young people.
• Support the delivery of trauma-informed, evidence-based care practices across services.

What You'll Bring

• Diploma in Community Services, Youth Work, Social Work (or currently working towards)
• Proven case management and stakeholder engagement skills
• Knowledge of trauma-informed and therapeutic care practices
• Strong documentation, organisational and compliance capabilities
• Current driver's licence and required probity clearances
