Lifestyle Solutions is seeking a dedicated role to support children and young people in Out-of-Home Care through case management, therapeutic planning, and frontline coaching across two parts of the role: 18 hours per week with frontline staff in homes and 20 hours per week for case work duties, planning, documentation, and stakeholder engagement.
