Job summary

Qualifications

• Excellent written and verbal communication skills.
• Strong analytical skills.
• A current Driver's Licence is required.
• Disability Worker Screening and Working with Children Blue Card eligibility required.

Responsibilities

• Case management support and development of early intervention strategies.
• Intake, triage and initial assessment of risk and eligibility.
• Maintain participant information in electronic data systems per policy.
• Develop community partnerships and referrals.
• Attend meetings and participate in supervision and professional development.
• Undertake outreach and community engagement activities.
• Contribute to program development and community education.
• Comply with legislation, agreements and quality management systems.
