Job summary

Central Queensland Indigenous Development Ltd (CQID) is seeking a Case Manager to support young people aged 15–25 who are transitioning from child safety guardianship. The role emphasizes therapeutic guidance, resources, and coordinated services across CQID’s footprint, with some travel to service delivery locations.

The position requires a current QLD Driver's Licence, Blue Card, First Aid and C, plus strong cultural awareness and child-protection knowledge.

Qualifications

• Holds a current C Class Queensland Driver's License
• Current Working with Children Check, 'Blue Card'
• Holds or is willing to obtain a First Aid and C
• PR Certificate
• An Awareness of Indigenous communities and cultures and the historical and contemporary issues relevant to the needs of Aboriginal and Torres Strait Islander clients
• An awareness of child protection issues, legislation and requirements and relevant issues specific to Indigenous children and their families
• Experience in providing a relevant support service to vulnerable Indigenous children and their families
• Experience in negotiating and partnering with diverse agencies and organisations that enable young people to achieve independence e.g. services, schools, social welfare, Court, Police, Legal and health agencies

Responsibilities

• Support and walk alongside the young person to enhance their capability to adapt and connect with goal-oriented supports and interventions that are aligned to independence focused outcomes
• Be an Active Partner in Care Coordination – advocate and inform assessment, planning and review processes as you journey with the young person to achieve their desired outcomes
• Lead the coordination and facilitation of organisation and service supports that are aligned to agreed goals
• Follow the documents technical Practice Manual procedures and all relevant standards and additional practice level directions given by Senior Case Worker, Practice Development Manager, Manager and/or General Manager
• Facilitate and support opportunities that enable young people to maintain and strengthen their cultural identity through meaningful cultural connections
• Complete all record keeping requirements for compliance, accountability, reporting, evaluation purposes and contribute to and participate in continuous improvement activities.
• Build respectful and professional relationships with service users, agencies, partners, community, colleagues and clients
• Work always to advance the reputation of CQID and undertake additional relevant duties as required
• Participate and contribute to the organisation’s compliance obligations under the ISO 9001:2015 Quality Management System and Human Services Quality Standards (HSQS)
• At all times work within CQID’s Workplace Health & Safety Policies and Procedure
• Other responsibilities as directed by the Manager and/or General Manager

Purpose of the Position

The Case Manager as part of Central Queensland Indigenous Development Ltd (CQID) organisation supports the achievement of CQID’s strategic goals of best practice and innovative alignment with progressive government reforms by effectively leading the professional practice within a specific portfolio of programs and services across the CQID service footprint.

The role applies a therapeutic approach to providing advice, resources and assistance to young people aged 15 years up to 25 years who are under or have transitioned from the Guardianship of the Department of Child Safety and who are on a journey to achieve their independence focused outcomes.

This role may be required at times to travel to relevant locations for service delivery or program/practice development purposes.
