Job summary

Community Access Care in Queensland is seeking an experienced Case Manager to lead the delivery of safe, high-quality, participant-centred services.

Reporting to the General Manager, you will mentor the Service Delivery team, oversee the participant journey from intake to exit, and ensure documentation and audits are consistently addressed.

This hands-on leadership role requires strong NDIS knowledge, excellent communication, and a commitment to confidentiality and continuous improvement.

Qualifications

• Experience leading or supervising teams.
• Strong understanding of NDIS principles and person-centred support.
• Experience with incident management and safeguarding.
• Excellent communication and organisational skills.
• Confidentiality and ethical conduct.

Responsibilities

• Lead safe, participant-centred and compliant service delivery across the region.
• Promote participant rights, dignity, privacy, cultural safety, choice and control.
• Oversee the participant journey from intake through to review and exit.
• Ensure records, support plans, risk assessments and Service Agreements are audit-ready.
• Oversee incidents, complaints, safeguarding and escalations.
• Monitor service quality, participant outcomes and budgets.
• Lead, mentor and support the Service Delivery team.
• Collaborate with Rostering, Compliance, Finance, HR and other teams.
• Support audits, quality assurance and data collection.
• Build relationships with stakeholders and represent CAC at events.
• Support strategic goals and service growth within CAC’s registration scope.
