Job summary

North West Queensland in Mount Isa, Queensland is seeking an on-site Social Worker to work with individuals, families and the wider community. Relocation support is available for the successful candidate.

Qualifications

• Bachelor of Social Work (or equivalent) is required and eligibility for AASW membership.
• Experience in case management, client assessment and support planning within community, healthcare or social services settings.
• Strong communication, interpersonal skills and ability to work with diverse communities.

Responsibilities

• Conduct client assessments and identify support needs at individual, family and community levels.
• Develop and implement appropriate support and care plans.
• Provide flexible, person-centred support across a diverse caseload.
• Deliver crisis intervention, guidance, advocacy and referrals.
• Connect clients with community services and government programs.
• Collaborate with multidisciplinary teams and community organisations.
• Maintain accurate, confidential case records.
