Job summary

Lutheran Services Graceville Centre, Nambour, QLD seeks a Full Time Case Manager for Disability and Mental Health to support individuals in attaining independence and wellness. You will apply evidence-based practices and coordinate care with families, carers and teams.

The role requires relevant qualifications, strong client focus, excellent communication, and proficiency with Outlook, Excel and Word; a current Australian Driver's Licence is also the norm.

Qualifications

• Tertiary qualification in a relevant Human Services discipline or current enrolment in a related program.
• Experience in the Community Services sector with client-focused, evidence-based practice.
• Strong communication, analytical and relationship-building skills.

Responsibilities

• Deliver comprehensive case management to support individuals to achieve goals.
• Apply evidence-based practice to develop person-centered support plans.
• Conduct regular reviews of client plans and monitor progress.
• Collaborate with multidisciplinary teams, families and carers for coordinated care.
• Maintain confidential documentation in accordance with standards.
