Lutheran Services is seeking a Part-time Case Manager – Disability and Mental Health at the Graceville Centre, Nambour, QLD. The role supports individuals living with disability or psychosocial disability in a recovery-focused framework.

You will deliver person-centered case management, use evidence-based practices, and coordinate with families, carers and multidisciplinary teams to improve wellness and independence.

Qualifications

• Tertiary qualification in a relevant Human Services discipline, or current enrolment in a relevant program.
• Proven experience within the Community Services sector or transferable experience.
• Exceptional interpersonal and communication abilities, with capacity to build effective relationships.
• Strong analytical and decision-making skills.
• Ability to perform in a dynamic, fast-paced environment.
• Proficiency in Outlook, Excel and Word; confident using mobile apps for case management and reporting requirements.
• Must hold and maintain a valid Working with Children Check (Blue Card), NDIS Worker Screening Clearance, and current Australian Driver’s Licence.

About The Role:

Located in Nambouron theSunshine Coast, ourGraceville Centreprovides support to individuals living with a disability and/or mental illness. All Graceville services are conducted by working with individuals in a strength based and recovery focused framework. Graceville Centre operates seven programmes that provide support to clients either on site or in their own homes. We are a growing service and regularly have internal opportunities for career growth.

As aCase Manageryou will work directly with individuals living with a disability including those with a psychosocial disability, using evidence-based practices to empower to live independently, maintain wellness, and enhance their overall quality of life.

Key Responsibilities:

• Deliver comprehensive case management to support individuals living with a disability or psychosocial disability.

About You:

The successful applicant will be a highly skilled and qualified professional with demonstrated experience in evidence-based case management and a commitment to empowering individuals living with disability and/or mental illness.

• Tertiary qualification in a relevant Human Services discipline, or current enrolment in a relevant program. Equivalent relevant experience may also be considered.


• Proven experience within the Community Services sector or transferable experience from a related industry demonstrating strong client-focused and evidence-based practice skills.


• Exceptional interpersonal and communication abilities, with the capacity to build effective, respectful relationships with clients, families, and multidisciplinary teams.


• Strong analytical and decision-making skills, with the ability to assess complex situations and implement practical, evidence-informed solutions.


• Ability to perform effectively in a dynamic, fast-paced environment, managing competing priorities while maintaining service quality and professionalism.


• Proficiency in Microsoft Outlook, Excel, and Word, and confidence using mobile applications to support case management and reporting requirements.


• Must hold and maintain avalid Working with Children Check (Blue Card),NDIS Worker Screening Clearance, and acurrent Australian Driver’s Licence.
