Job summary

Lutheran Services is seeking a Senior Case Manager for the Empowering Family Connections program in Caboolture, QLD. This part-time role contributes to delivering end-to-end case management for young people and families within a not-for-profit community services setting.

You’ll work with Child Safety, Youth Justice, Health and Education partners to support home stability and life skills, including risk assessments and tailored support plans.

Qualifications

• Degree in Human Services, Social Work, Psychology or related field.
• Experience in case management within youth, family or community services.
• Ability to manage complex cases using therapeutic and evidence-based approaches.
• Confidence working with multiple stakeholders and high-risk situations.
• Excellent communication, documentation and organisational skills.

Responsibilities

• Deliver end-to-end case management for young people and families.
• Conduct risk and needs assessments and develop tailored support plans.
• Provide counselling, mediation, advocacy, and family support.
• Support home stability, life skills development, and transitions.
• Collaborate with Child Safety, Youth Justice, Health, Education and other stakeholders.
• Facilitate individual and group programs.
• Maintain accurate case notes, reporting, and data collection.
• Participate in community networks and service development initiatives.
