Job summary

WA Country Health Service is seeking a position to centrally coordinate services across country and metropolitan WA for transitional care for older people, screening referrals to ensure timely discharge and appropriate options.

You will navigate older adults' pathway from hospital into community care, working with a multi-disciplinary team and ensuring confidentiality and high standards of written communication.

Qualifications

• High level analytical and conceptual skills.
• Demonstrated ability to work in a multi-disciplinary team independently and proven ability to communicate with and gain the cooperation of staff at all levels.
• Demonstrated high level written communication skills including report writing.
• Extensive administrative experience and highly developed time management skills.
• Demonstrated high level of computer literacy with experience using a wider range of computer software applications including databases.
• Demonstrated ability to always maintain confidentiality and exercise discretion.
