Job summary

You will drive partnerships, system-wide patient access, and discharge planning, while developing the workforce and promoting a values-based culture across the organisation.

Qualifications

• Clinical qualification with AHPRA registration or eligibility.
• Experience leading multidisciplinary teams in complex healthcare environments.
• Strong capability to build a positive, values-based workplace culture.
• Experience in contract management and working with associate providers.
• Expertise in clinical governance, quality, risk management and compliance.
• Operational and financial management including budget oversight.
• Experience driving patient access and flow improvements.
• Data-driven decision making with benchmarking and performance metrics.
• Proven ability to lead continuous improvement and change initiatives.
• Commitment to consumer partnership and co-design across care continuum.
• Experience in Transition Care and/or aged care services.

Responsibilities

• Provide operational leadership across community and bed-based transition care.
• Oversee governance, workforce management, financial performance.
• Build partnerships with internal teams and external providers.
• Ensure services meet quality, safety and compliance standards.
• Lead workforce planning, capability development and performance frameworks.
• Drive system-wide patient access and timely intake and discharge planning.
• Foster a positive, values-based culture and staff engagement.
