Job summary

Central Adelaide Local Health Network (CALHN) is seeking a Placement Officer to support older people transitioning from hospital to residential aged care. You will coordinate applications, assist discharge planning and collaborate with patients, families and providers to find care options aligned with individual needs.

The role requires experience in acute, subacute or aged care settings, strong empathy, and competence in Microsoft Office.

Qualifications

• Excellent empathy and communication skills for patient-centred care.
• Ability to navigate complex discharge planning and multi-disciplinary collaboration.
• Strong written and verbal communication to advocate for patient preferences and budgets.
• Proven ability to maintain accurate and secure records in busy health settings.
• Experience in aged care, subacute or acute care environments is desirable.

Responsibilities
