Job summary

My Guardian in Sydney Olympic Park is seeking a full-time on-site Care Coordinator to support aged and disability services. You will roster staff, liaise with clients and families, and ensure high-quality, person-centred care in a fast-paced environment.

You will work with Case Managers on care goals, assist with NDIS coordination, manage service delivery and budgets, and help maintain smooth office operations.

Responsibilities

• Coordinate staff rostering and scheduling to ensure shifts are covered.
• Build relationships with clients and families for high-quality service.
• Provide on-call support on a rotating roster for urgent changes.
• Work with clients, carers and Case Managers to meet care goals and outcomes.
• Assist with NDIS and aged care coordination, including service delivery and budgets.
• Coordinate client-related services and resources, including maintenance and supplies.
• Support office operations, including records management and communication.
