Job summary

Uniting AgeWell in Ballarat is seeking a Care Organiser to coordinate and schedule services across our Support at Home and CHSP programs. You will match care staff to client needs, manage schedules, respond to changes, and ensure high-quality, person-centred care.

This full-time role operates Monday to Friday from our Ballarat office, requires strong organisation, excellent communication and the ability to prioritise.

Qualifications

• Experience coordinating and scheduling services within aged care, health or community settings.
• Strong organisation and time management; ability to prioritise
• Excellent communication and customer service to build relationships with clients and families
• Proactive, solutions-focused approach and adaptability to changing priorities

Responsibilities

• Coordinating and scheduling services across Support at Home and CHSP
• Matching care staff to client needs and availability
• Managing service schedules and responding to changes, cancellations and urgent requests
• Liaising with clients, families, Care Advisors and staff to ensure continuity of care
• Monitoring service delivery and adjusting schedules as needed
• Maintaining accurate client records and scheduling information
• Responding to enquiries with exceptional customer service
• Ensuring scheduling processes comply with policies and funding rules
