Uniting AgeWell Limited is seeking a Care Organiser in Ballarat to coordinate and schedule services across CHSP and Support at Home programs. You will work closely with clients, families, Care Advisors and staff to ensure timely, person-centred care and smooth operations.

The role involves matching staff to needs, managing changes, responding to urgent requests, and maintaining accurate client records with strong attention to detail and a service-first attitude.

Qualificações

• Experience coordinating and scheduling services within aged care, health or community services.
• Strong organisational and time management skills, with the ability to manage competing priorities.
• Excellent communication and customer service skills, with the ability to build positive relationships with clients, families and colleagues.
• Proactive, solutions-focused approach with the ability to adapt to changing priorities.
• High attention to detail and accuracy when maintaining schedules and client records.

Responsabilidades

• Coordinating and scheduling services across the Support at Home and Commonwealth Home Support Programme (CHSP).
• Matching care staff to client needs, preferences and availability.
• Managing service schedules and responding to changes, cancellations and urgent requests.
• Liaising with clients, families, Care Advisors and support staff to ensure continuity of care.
• Monitoring service delivery and making scheduling adjustments to meet client needs; maintaining records.
